Abstract

Due to the spread of the COVID-19 pandemic, face-to-face educational events and classes were transferred to a distance format, so the traditional II All-Russian Olympiad with international participation in therapy was also held in absentia on April 26-28, 2021. This format provided the opportunity for 759 students and residents to participate in it from 82 educational and scientific organizations in Russia, Kazakhstan, Uzbekistan, Tajikistan and Belarus. This article analyzes the Olympiad held in order to study the possibility of assessing professional competencies using distance technologies among students of 4-6 courses and residents of a therapeutic profile. As tools for assessing the formation of competencies, we used: testing knowledge using an electronic platform for remote testing and assessing practical skills using a computer screen simulator Body Interact. The analysis of the results showed that the bank of test tasks and clinical cases is valid, reliable, and can be used to assess the professional competencies of students in specialty and residency programs of a therapeutic profile.

Abstract

An analysis of the experience of using the interactive application "Academics 3D" in the context of the COVID-19 pandemic in teaching students of the medical faculty is presented. Virtual technologies make it possible to optimally distribute the time limit and stages of mastering practical skills in the professional training of doctors. The analysis showed the significance, prospects and ways of increasing the efficiency of using simulation technologies in the process of training doctors.

Abstract

The aim of the study is to assess the effectiveness of the development of communicative competence among residents of the Department of Obstetrics, Gynecology and Perinatology using a series of psychological trainings. According to the results of post-tests, there was an increase in reflexivity in 50% of respondents, as well as a change in the level of empathy. There was a transition from the subject-object relationship with the patient to the subject-subject relationship, which made it possible to hold patients more accountable for their own health and the health of their future children. The developed training will be used in the work with young specialists.

Abstract

The main stages of the perioperative period in acute appendicitis were recreated in simulated conditions with 5th year students: differential diagnosis; preoperative preparation; diagnostic laparoscopy; laparoscopic appendectomy; suturing an operating wound; postoperative management. The entrance testing and the final questionnaire were carried out. The high efficiency of classes and high motivation of students to this format of training are shown, which gives grounds for a wider use of simulation classes in teaching clinical disciplines to senior students.

A study was carried out among 76 students who took part in the Olympiad contests in practical medical skills in an innovative distance format. The data obtained testify to the effectiveness of distance simulation training, the assessment of which was carried out by students answering test questions before and after passing the test task, as well as by assessing the students' actions during the remote passage of the simulation scenario by experts.

The purpose of the work was: the development of a non-costly, effective method for assessing students' knowledge, which would stimulate the interest of a medical student in the educational process. The analysis of 19 videotapes of laparoscopic cholecystectomy performed by accredited on the virtual simulator "LapSim" during the passage of the station "Planned Surgery" within the framework of primary specialized accreditation in the specialty "Surgery" was carried out. The mistakes made by those accredited in the application of high-energy methods at the stage of isolation of the elements of the hepatoduodenal ligament have been studied. Mistakes when using high-energy methods were made by 14 out of 19 accredited. The importance of training surgeons in the basics of electrosurgical safety is emphasized.

An important feature of the simulation training of residents-cardiologists is that students have the opportunity to hone their skills and abilities in the center as soon as possible, using modern simulators, while in practical medicine this would take a much longer time. Of course, simulation conditions within the framework of simulation training will not completely replace practical experience with real patients, but definitely, this allows you to increase the level of professional training of cardiologists.

According to the current legislation medical institutions of any specialization and form of ownership are obliged to provide emergency medical care without delay and free of charge. Emergency medical care is not a profile one for many specialists and, within the framework of the current law, it is necessary to introduce an emergency medical care course into the CME system for all medical specialties at least once every five years, which will correspond to both the previously adopted system of postgraduate education and the existing standards of work of medical facilities.

For the first time, they started talking about virtual simulation technologies in our country in 2002, when at the Institute. Vishnevsky, a virtual simulator "LapSim" was presented at the Congress of Endosurgeons. Since then, there have been great changes in the development of simulators, mannequins and simulators, teaching methods in preclinical training of students. The assessment of communication skills in the accreditation procedure is carried out in simulated conditions with the participation of a standardized patient (SP). SP is a clinical history simulator. A standardized patient in accreditation is a standard of behavior: the same conditions for all accredited, strict adherence to the clinical scenario, a dosed story of the scenario strictly in accordance with the accredited’ communicative actions (actions according to the checklist). A study was carried out among 117 students of the medical faculty of the Amur State Medical Academy of the Ministry of Health of Russia, who were trained under the program of the discipline "Emergencies in the practice of a district therapist" within the framework of the curriculum. The data obtained indicate the effectiveness of the new remote method of preparation for the practice of CPR. The average value of the percentage of chest compressions (CHC) performed with the correct frequency in the group of students trained according to the proposed method is 34.1% more than among students preparing according to the traditional method.

The training of transosseous osteosynthesis for traumatologists-orthopedists is actual. Purpose of the research: development of OSKE stations in teaching the method of transosseous osteosynthesis. Material and methods. In the course of the simulation the technique of passing the spokes through the synthetic bone, the installation of the apparatus and the biomechanics of control are practiced. Results. The developed OSKE stations allowed us to objectively assess the level of training within a specified time frame. Conclusion: OSKE is an effective tool for the control of knowledge, including the methods of transosseous osteosynthesis. The inclusion of transosseous osteosynthesis station in the accreditation of orthopedic traumatologists is an urgent task.

Practical preparation for primary specialized accreditation is not always available to the trainees of the professional retraining cycle. Most of the specialists undergo independent training using video tutorials posted on Internet resources. The analysis of the quality of the video manuals created according to the passport of the OSCE "emergency medical aid" station in the most cases showed the formality of the implementation of practical skills in the provision of emergency assistance. The created software shell consists of three functionally different modules: creating and editing video conferencing; use of videoconferencing; monitoring of the educational process. The developed software shell has remote access and can become the main tool for the repository of virtual computer simulations. Currently, videoconferencing in the field of cardiology has been created as a potential digital base for practical clinical studies and accreditation.

Intracorporeal laparoscopic suture (ICS) is an important skill of the surgeon in performing laparoscopic procedures. Simulation training in this skill contributes to the formation of a skill and its implementation in operational practice. There are not enough publications highlighting the effectiveness of simulation training of the skill from the point of view of application in practice. Interviews were conducted with surgeons who underwent such simulation training in order to determine the proportion of doctors who applied the acquired skill in a real operating room. After training 88.9% of surgeons started using Intracorporeal laparoscopic suture; intracorporeal sliding knot — 77.3%.

Making a simulation trainer from a student has been described earlier. When analyzing the training experience (training of trainers) of the teaching staff of universities, two groups were identified. During the training we faced a number of problems. The difficulties that have arisen we explain by the preservation in the education system of a large share of information teaching methods and orientation to the hourly component of disciplines. A step-by-step system of training trainers of simulation training from existing teachers is justified if there is a voluntary involvement in this process.

The work assesses the psychological readiness to provide first aid in everyday life, assesses theoretical knowledge on cardiopulmonary resuscitation and assesses practical actions for providing first aid in simulated conditions on the basis of the Far Eastern State Medical University.

The study involved 44 people. 59.09% were trained at various stages of their life in providing first aid. According to the respondents, 27.27% are confident in the skills of first aid. Complex correct mastery of first aid techniques was demonstrated only by 4 people (9.09%).

This article discusses the logic and aspects of creating a web application for the accumulation and processing of incoming data of radiation diagnostics and their transformation into information that can be used by students of medical educational institutions in training in the absence or lack of anatomical materials. In prospective analysis, it is possible to use this software product not only as a means for simulation training, but also in medical applied practice in preoperative surgical planning, modeling various mechanical characteristics of implants.

The development of medical science and the technical support of hospitals leads to the annual appearance of new instruments in the hands of the cardiovascular surgeon. The improvement and expansion of the series of X-ray endovascular catheters and electrodes, as well as the differences between similar models from different manufacturers, forces the doctor to either take a new instrument into the operating room without prior approbation with the risk of reducing the effectiveness of treatment or use old, familiar equipment, which reduces the possibility of using the entire range of consumables and prevents personalization medical care.
