THE PREPARING OF PERSONNEL FOR SIMULATION TRAINING
On April 6, 2017, the first training course for teachers and other employees of simulation centers with the title “Specialist in Medical Simulation Training“ was held. Since the beginning of the program, about 670 ROSOMED certificates have been issued to participants who have successfully completed such training in various simulation centers. This publication analyzes the responses of 293 respondents of pre-training survey, 261 participants in free practice discussions, 1749 practice test attempts, and 7 anonymous statements via a special form. As a result, an updated program will appear in the new academic year under the same conditions.
On the example of a clinical case, we used software 3D modeling of the reconstruction of the shape of the skull of a child aged 2 years and 5 months with metopic craniosynostosis, as well as manual training using a craniotomy on an individual 3D model of the patient's skull before surgery. The use of a plastic 3D model of a particular patient made it possible to conduct a “trial, training operation” to work out the optimal osteotomy geometry. An intraoperative evaluation of the result obtained on a visual scanner was also carried out.
Purpose of the study. Improving the result of surgical treatment of a child with metopic craniosynostosis using preoperative 3D planning and personalized simulation of a planned operation. For the purpose of preoperative planning we used an algorithm with the inclusion of 3D technologies: preoperative modeling on software, simulation of surgery on plastic models of the skull, surgical intervention along craniotomy lines, intraoperative visual scanning and assessment of the results of skull reconstruction.
Conclusion. The preoperative 3D modeling of the skull formed a complete plan for individual reconstructive changes in the skull in our patient with metopic craniosynostosis over the age of two years, and allowed us to perform a “trial, training operation” on a plastic model to work out the optimal osteotomy geometry. This made it possible to provide the optimal technique for a real operation, speed up its implementation, reduce trauma and blood loss, the duration of anesthesia, and, in combination, improve the results of surgical treatment.
In medical universities of the Kyrgyz Republic, the final stage of education is the passing of the Final State Attestation, which establishes the level of readiness of graduates to perform professional tasks and its compliance with the requirements of the State Educational Standard for Higher Professional Education of the Ministry of Education and Science of the Kyrgyz Republic in the specialty 560001 — “General Medicine“. The article summarizes the experience of conducting the Final State Attestation of graduates of the international medical (IMF) and medical faculties (MF) of Osh State University using the “Multidisciplinary University Virtual Clinic DIMEDUS” as one of the elements of the assessment tools, and also presents the results of an online survey, where graduates of MMF and MF for the 2021-2022 academic year gave their assessment of the Final State Attestation using the DIMEDUS virtual clinic and the prospects for using virtual technologies in training and certification in the future.
The concept of “distance learning” in the first year implies not only the actual system of knowledge assimilation using electronic resources, but also the organization of a novice student, in particular, his familiarization with the types of learning activities — lectures, practical classes, forms of knowledge control, the content and scope of independent work, the rules of its performance, illustration material. Of particular importance is the installation of scientific and educational and research work (SRW, ERW). The introduction of distance learning elements appeared long before the pandemic COVID-19 in the MOODLE variant. The work illustrates the experience of using this system when studying the discipline of Biology, including the topic “Research Methods in Human Genetics” of the section “Fundamentals of Human Genetics”. The dynamic educational environment and its management system based on computer and Internet technologies proved to be important learning tools for first-year students especially during the transition to a distance learning format. Along with the traditional face-to-face forms of classes and teaching and research work, it is reasonable for each student to use MOODLE content, work on Internet platforms Skype, Zoom in virtual contact with the teacher and his/her active methodological role.
By the end of the training, clinical residents in the specialty anesthesiologist-resuscitator should have sufficient skills in prosthetics of the upper respiratory tract. Considering that a large number of complications occur during tracheal intubation, and the prolongation of this stage significantly increases the frequency and severity of complications, it is necessary to focus on quick and safe tracheal intubation in training. This article presents the results of an analysis of the effectiveness of training resident anesthesiologists-resuscitators in tracheal intubation using simulation techniques.
The features of simulation training in CPR in the framework of the student scientific group of the Department of Disaster Medicine of N. I. Pirogov Russian National Research Medical University, as well as data on this topic in the period from 2019 to 2022 were studied in this article.
